How To Apply

Referrals are accepted from substance use counsellors (Fraser Health direct service and contracted service providers including Creekside Withdrawal Management), youth concurrent disorders therapists, and Fraser Health early psychosis Intervention program therapists. Referrals are also accepted from the following non-substance use providers who agree to function as the case manager for the youth: child and youth mental health therapists, social workers, clinical counsellors, and school counsellors.

There are two types of forms:
Referrals for Substance Use Providers
Referrals for Non-Substance Use Service Providers

Please only complete the form if your participant meets the eligibility requirements. If you have questions, please call or email us.

Referrals must be completed electronically and emailed to dewyprogram@pcrs.ca.

Once completed referrals have been received by email, they will be forwarded to the Fraser Health Screening Committee for review. Once screened, DEWY staff will contact the youth directly to set up an intake meeting.

Counsellors are available to provide program orientation visits prior to, or during, the referral process.
