Family and Youth Information: Support for FASD and Complex Needs

The FYI Program provides support to families with youth, aged 12 - 18 years, affected by Fetal Alcohol Spectrum Disorder (FASD) and other complex needs.

The FYI Program Social Worker provides information, support, parenting strategies, service coordination, partnership, and advocacy to enable families to provide enhanced care to their youth. 

The FYI Program works in partnership with families to assist them to meet the needs of FAS affected youth.

PROGRAM FEATURES

The FYI Program assists families to have strong and comprehensive care teams set on meeting the unique needs of each individual family.  It provides consultation to community services to enhance knowledge of the effects of FASD.  Please contact the program's social worker to arrange consultation appointments. 

 

WHO IS ELIGIBLE?

Families who have a youth with a diagnosis in the FASD range or other Complex Developmental Behaviour Conditions are appropriate for FYI Program services.  

• The geographic boundaries of the FYI Program are: east of 120th Street (Scott Road), west of 200th street, north of Highway-10, and south of the Fraser River, Surrey BC

• Families with youth who are on the FHAN waitlist for assessment are eligible for FYI services

ACCESS
If a youth has been diagnosed with an FASD or Complex Need, referrals can be made directly to the FYI Program Social Worker.
If the youth has not been diagnosed, referrals can be made to Sunny Hill Hospital's Assessment Network by the youth’s paediatrician or child psychiatrist.  Once the referral for assessment has been made, the family is eligible to access Key Worker Services.
HOURS AND LOCATION OF SERVICE 
Hours of service are flexible--Monday through Friday.
Services are provided either on-site or outreach, according to family need.
WHAT CAN I EXPECT IN THIS PROGRAM?
90% of the families in this program in 2009 reported progress on their individualized goals which include access to information, support, advocacy and help with services coordination/partnership.
